Market case study · Aging practice identification
Which physicians near Springfield may be thinking about retirement
Here's the physician practice tool worked through for one market: every physician within 25 miles of Springfield, Massachusetts, by estimated age and by how likely they are to practice outside a hospital system. Names and addresses are greyed out. A licensed account sees them, along with NPIs and a CSV.
1. Start with the market's age curve
The tool opens on the share of the area's physicians in each age band, against the nation (dashed). Primary care here is 40% aged 55 and over against 32% nationally. A market whose curve sits above the nation's has more practices that will change hands in the next decade.
vs 32% U.S. (+8 pts)
vs 32% U.S. (+1 pts)
vs 35% U.S. (+4 pts)
vs 30% U.S. (+3 pts)
Solid line: Springfield. Dashed line: United States. Each curve is the share of that group's physicians in each age band.
2. Filter to age, specialty and independent practice (likelihood)
Each physician gets an independent practice likelihood from public CMS records: high, medium or low. High means we found no tie to a hospital. Whether a physician plans to retire is a question for the call.
Independent practice: high
34
No hospital tie found. Aged 60 to 80 and billing Medicare.
Independent practice: medium
264
Hospital privileges on CMS, or a mostly hospital-affiliated billing group. Aged 60 to 80 and billing Medicare.
Independent practice: low
354
At a hospital or system address. Aged 60 to 80 and billing Medicare.

A sample of 14 of the 31 high-rated physicians aged 60 to 80 in billing groups of five or fewer. The licensed list shows the name, NPI, exact estimated age, street address, billing group name and Medicare volume for each. Specialty is shown as a group and volume as a band here.

How to use it
- Start from the 60+ physicians with a high independent practice likelihood in a solo or small group: the practices nearest to a transition.
- Check the medium-likelihood physicians next. Privileges at a hospital are common for independent physicians, so they are leads, not exclusions.
- Download the licensed CSV for the area and match it against your own list before outreach.
Ages are estimated from medical school graduation year (minus 26) and run young for international graduates. The list covers physicians enrolled in Medicare; those who see no Medicare patients are not in it.
